Food access confidentiality agreement

Thank you for volunteering with us at Three Sisters! We appreciate your time and investment in our food access mission: helping improve access to fresh, local produce for our neighbors. Respecting that some information is personal and sensitive for the people we work with, we are asking our volunteers to please read and fill out this confidentiality agreement.

As a volunteer helping deliver produce to local households and working with Three Sisters’ CSA members, I agree to:

  • Treat all community members with respect and dignity;

  • Keep confidential Three Sisters’ CSA members’ personal information (household income or economic status, as well as phone numbers, home address, or other contact information).
